Building Bridges

`Building Bridges` is a registered domiciliary service for people with learning disabilities.

`Building Bridges` is for people that live in their own homes, or with their family, and want to receive extra support so that they can be more independent.

Services that we provide.

`Building Bridges` will be pleased to support you with:

  • Personal care
  • Skills training
  • Daily living skills
  • Leisure activities

We can support you at a time of your choice and for as long as you need every day of the year.

People that we will work with.

Any adult over the age of 19 that has a learning disability and may also have:

  • Health care needs
  • Physical disabilities
  • Sensory disabilities
  • Behaviours that others describe as challenging
  • Children of their own.

How we work with people.

  • WE TAKE YOUR REFERRAL.
    You can refer yourself or ask someone else to do this for you.
  • WE ASSESS YOUR NEEDS.
    We will come and talk to you about yourself and what you need support with, someone else can be there to help you with this.
  • WE WILL HELP YOU WITH A RISK ASSESSMENT.
  • WE WILL HELP YOU MAKE UP A SUPPORT PLAN.
    This will have all of your support needs in it and instructions on how these are to be met. This is your plan and will be kept in your home.
  • REVIEWS.
    Because you will be getting more independent, your needs will change. This is great, all your hard work will have worked for you.

Your review will be Person Centred and will be for you.
